Registration Requirements and Performance Evaluation Key Points for a Nine-Pathogen Respiratory Nucleic Acid Detection Kit (Fluorescence PCR Method)
Product Classification and Registration Pathway
According to the "Catalog of In Vitro Diagnostic Reagents," a nine-pathogen respiratory nucleic acid detection kit (fluorescence PCR method) generally belongs to Class III in vitro diagnostic reagents and must undergo registration approval by the National Medical Products Administration (NMPA). Applicant companies shall submit complete documentation in accordance with the "Measures for the Registration Administration of In Vitro Diagnostic Reagents," including product technical requirements, analytical performance evaluation reports, clinical evaluation data, stability study data, etc. At the same time, they must meet the relevant requirements of the "Quality Management System for Medical Device Manufacturing" to ensure compliance throughout the entire process from design and development to production.
Core Indicators of Analytical Performance Evaluation
Analytical performance evaluation is a key module in the registration documentation. For this kit, validation data in the following aspects must be provided: 1) Limit of Detection (LoD): Use wild-type or standard bacterial/viral strains, determine the lowest detectable concentration through serial dilution, and generally require coverage of common clinical variants of each target. 2) Analytical specificity: Evaluate cross-reactivity with closely related pathogens (such as other respiratory viruses and common colonizing bacteria), while verifying the effects of endogenous interfering substances (such as blood, mucus, antibiotics). 3) Precision: Including intra-batch, inter-batch, intra-day, inter-day, and inter-operator variation, usually evaluated by Cq values or positive detection consistency. 4) Inclusivity: Use samples of different types/subtypes to verify amplification capability. 5) Nucleic acid extraction efficiency and residual inhibitors: Comparison data with reference methods shall be provided.
Clinical Study Design and Sample Requirements
Clinical studies shall adopt a prospective or retrospective design, and samples shall come from the target population (patients with suspected respiratory infection). Comparison shall be made with marketed products of the same type or gold standard methods (such as culture, sequencing). The sample size shall be calculated according to statistical requirements to ensure sufficient positive cases for each target. For rare pathogens (such as Chlamydia pneumoniae), laboratory simulated samples or third-party quality control materials may be accepted. Study evaluation indicators include clinical sensitivity, clinical specificity, positive predictive value, negative predictive value, and overall agreement rate. At the same time, attention shall be paid to the detection accuracy of mixed infection samples.
Stability and Transport Validation
The stability of the kit includes long-term stability, accelerated stability, open-vial stability, and freeze-thaw stability. Observation shall be conducted for at least 6 months to 1 year under the specified storage conditions (such as -20±5°C). Transport simulation tests shall cover the effects of extreme temperatures (such as thermal shock and vibration) on reagent performance. Sample stability also needs to be verified, including the effects of different preservation solutions, storage times, and freeze-thaw cycles on nucleic acid integrity. All data shall be documented in reports, clearly specifying the shelf life and conditions of use.
Quality Management System Requirements
Manufacturing enterprises shall establish a quality management system compliant with GB/T 42061 (ISO 13485) or the "Good Manufacturing Practice for Medical Devices." Key control points include the design and synthesis quality of primers and probes, incoming inspection of raw materials, environmental control during production (to avoid aerosol contamination), and release testing of finished products. Each batch of products shall undergo factory release testing, including positive agreement rate, negative agreement rate, precision, etc. In addition, post-market adverse event monitoring and periodic risk evaluation shall be carried out to ensure the continued safety and effectiveness of the product.
